Support with medication prompting and administration in accordance with an individual's agreed care plan and medication requirements. We can also support people with attending GP, hospital and other healthcare or community appointments where this forms part of their care arrangements.

Companionship & Wellbeing

Friendly and meaningful companionship to promote emotional wellbeing, reduce loneliness and help people remain socially connected. Our welfare and wellbeing visits also provide regular reassurance and practical support according to individual needs.

Meals, Shopping & Home Support

Support with preparing meals and drinks while encouraging good nutrition, hydration and individual dietary preferences. We also assist with shopping and light household activities to help maintain a safe, comfortable and welcoming home environment.

Mobility & Community Access

Assistance with safe movement around the home, transfers and mobility while encouraging individuals to maintain their abilities and independence. We also support people to stay connected with their communities, visit family and friends, attend activities and continue doing the things that matter to them.

Respite & Hospital Discharge Care

Flexible support for families and unpaid carers who need a break, while ensuring their loved one continues to receive appropriate care. We also provide short-term or ongoing home care following hospital discharge, helping individuals safely rebuild confidence and independence at home.
